Triloculotrema japanicae n. g., n. sp. (Monogenea: Monocotylidae: Merizocotylinae) is described. The parasite inhabits the olfactory sacs of the Japanese topeshark Hemitriakis japanica (Carcharhiniformes: Triakidae), and possesses a distinctive haptor with only three loculi. Some comments are made on the evolution of monocotylid monogeneans.
